Well since I am a one-person wrecking ball with a personal mission to wipe political correctness off the face of the Earth, I think "First Lady" is a suitable title for ceremonial occasions. She is properly formally addressed as Mrs. (whatever.) Former first ladies are also addressed as Mrs. Reagan or Mrs. Carter or Mrs. Bush. In Hillary's case she quickly morphed from Mrs. Clinton to Senator Clinton shortly after she was no longer first lady so with her it was not an issue.
In America, the proper title to address former Presidents still living is "Mr. President" even though they are out of office. We do the same with former speakers of the House--Newt Gingrich is still routinely addressed as Mr. Speaker. Nancy Pelosi is addressed as Madam Speaker. Once a Congressman always a Congressman and once a Senator always a Senator when they are being formally addressed.
So now we have the awkward situation in which a former President could be the first gentleman. But to refer to him as Mr. President does create a confusing situation but it seems improper to call him First Gentleman or Mr. Clinton.
I suggest we elect Trump.
