I suspect there are no surgeons without without a 4-year undergraduate degree in the United States, but I bet there are at least a few 4-year undergraduate degree holders that are not surgeons. Many people that work at a trade are educated. They may have read widely, discussed books and subjects with others, traveled, gone to museums, read at libraries, taken courses, used their knowledge to enhance their professional work. They may have gone to college and studied something quite different from their vocation. What they haven't done is ignore curiosity and new ideas. They are educated.
