"Trans women in sports" is just the most extreme example of an LGBT-etc ideological stance that many find harmful to society. It's not just what people do in private, it's about what ideologues try to change about society's rules.

A transgender female is a trans man because that's the only way you can be female in that situation.

It's representative of that organization which isn't even an exclusively western one and that organization does indeed recognize gender as a social construct and allows trans people to compete as the gender they identify with provided they were on hormone blockers and didn't go through male puberty. Disorder is the term that biologists and health professionals use because defective implies that their bodies were made wrong rather than just different than how we normally see them ordered.
