If I missed this point please forgive me but why not take all non-medical professionals (such as insurance company personnel & government bureaucrats) OUT of the role of decision maker for what medical procedures & tests will be paid for?
Have a panel of medical doctors ONLY, openly make such decisions & only make patient's privacy considerations a reason for not making their deliberations public. (their decisions could be open to a quick appellate review (by a medical review appellate board) in the event a patient has a legitimate complaint about an initial decision.....kinda like the FISA court is set up for quick actions involving fgn intelligence surveillance problems)
Right now, insurance company accountants are practicing medicine without a license, in my view. Crazy!

I also agree with some medical tort reform, even though I think said reforms would ultimately have little effect on overall HC costs. I agree with it just because it makes sense to me in that it would be a small step in the cost savings direction.
