At "conception," all that is made is a zygote, which grows into a blastocyst that implants. You still don't know if it is twins or triplets, and even if it is, you don't know how many persons there will be.
When a woman gives birth to twins, there can be one genetic code but two bodies that house two persons. If she gives birth to conjoined twins with only one body, if it has only one functional head, there is only one person, and if there are two functional heads, there are two persons. This is only determined at birth.
