Of course there has been. How did the early settlers on the edges of civilization get healthcare? How did American citizens in 1801 get
healthcare? Never forget the earliest hospitals where not government run, they were owned and run by churches.

An essential part of a free market is that it is not controlled by government. While the earliest religious hospitals where motivated by charity rather than economics they were most certainly a non-governmental response to a need for healthcare. They played a critical role in the development of the entire American healthcare system

The purpose of government
The definition of freedom as it relates to a civil society.
The history and development of capitalism and the definition of free market
Life, death, freedom and medical systems in the 16th, 17th, 18th, 19th, 20th and 21st centuries
The history, development and control of the US medical system by the AMA
The impact of church run hospitals on the free market system
History of pharmaceuticals and the development of pharmaceutical corporations.
Abortion statistics and laws
The laws impacting interstate travel by private citizens
Women.
Poor women
Poverty

Exactly what is it that think other posters don't understand?
