There never was a free market in medical services. It is not just a random coincidence that medical costs in the US are too high for a huge chunk of the population. Since 1880 the AMA has been working overtime to limit the number of medical schools, limit the number of doctors and raise the cost of their services. The AMA never in all their history considered serving everyone equally. Starting with passing a law that midwives were no longer allowed to deliver babies because they were ignorant, unprofessional, uneducated, and dirty and denying women admittance to medical school. An essential part of a free market is that it is not controlled by government. While the earliest religious hospitals where motivated by charity rather than economics they were most certainly a non-governmental response to a need for healthcare.
