The result has been more hospitals owned by insurance companies and such, and concentrated into conglomerates creating monopolies that jack up the costs and reduce the level of patient care.

Say a hospital is given to a group of doctors by the insurance company that owns it. Now the doctors have to buy their insurance (an enormous part of the cost of health care) from the insurance company that gave it to them.
Doesn't it seem to you like taking the cost of insurance out of the equation would reduce the cost of hospital treatment?
After all, some surgeons in the US have to earn hundreds of thousands to cover their malpractice insurance, before they pay staff and rent and put anything in their pockets.
