Why would that be a difficult thing to admit? Why can't he say that was the personal choice he made but others may choose differently if keeping the base is so important to him? My bet is that the fact his major donor is heavily invested in a particular therapeutic means saying anything that may prevent disease is not in his political best interest. Just another example of how irresponsible he is.
