If it amounts to the flu, he can be eased back from public appearances and trotted out every now and again and lie to the people that he doesn't have it.
If COVID-19 forces Trump into hospital care, the WH would have to transfer duties to Pence, which would be made public.
BTW, given that many presidents over the years have lied about ailments, or downplayed them and in some cases outright lie...I won't get bent out of shape if the first instance happens.
