Russia perceives NATO expansion, both in adding member states as it's done 11 times in the past dozen years or so, as well as it's plans of deploying more MDS as a threat to them and they are going to act according to that. Putin is not the Czar of Russia and if he ordered a nuclear attack the result would, instead, be his assassination (likely presented publicly as his having a heart attack.) The structure of the Russian military is not like that of the USA. It is far more independent of the civil government.
Putin is dangerous in terms of regional expansion goals and potentially causing troubles in various countries. But he is not a threat of nuclear war. He does not have the power to unilaterally order a nuclear strike.
