Your ninja would have had to slip inside right behind the exiting pilot, before the door closes again (because the FDR records that) and then hit the lock, still unnoticed so the first officer doesn't make a sound. Then he has to render the first officer unconscious.... without making a sound or changing the FO's breathing patterns in the least. And then has to climb into the captain's seat to reach the autopilot controls. A task definitely not possible to do silently, I don't know if you've ever looked into an airliner cockpit. Take a peek sometime.

Disgruntled pilot doesn't equal suicidal pilot. I he was suicidal to begin with being disgruntled could be a factor in taking so many people with him, but if he wasn't suicidal I don't see how being disgruntled equals 150 dead people.
