This is the most likely. After all the bashing we heard about Biden's age over the last few years we forget Trump has started his second term just as old.He's old. Fragile. And if he's prescribed blood thinners, even a baby aspirin a day, bruises like this can show up unexpectedly in old people.

It could be something as simple as receiving IV antibiotics. He may have been unwell and received IV antibiotics - they are used for many situations and can be administered directly into a vein to enter the bloodstream pretty much straight away. Quicker and at a higher concentration. It's really not uncommon to see that type of bruising.Chemo?
