so show me - what example of a food pricing can be compared to how the disparity in drug pricing is ?

No, it isn't. You're confusing the US with the nations that have low drug prices.

Medicare does have a lot of purchasing power... but only for Medicare recipients.

Medicaid's payments for pharmaceuticals is not set by a king. They're determined by complex negotiations at both federal and state levels, and is totally separate from Medicare's negotiations. I.e. it's all but designed to make it impossible for one entity to utilize that leverage.

And even if Medicare and Medicaid could negotiate in unison, it'd still be around 20% of the population. That gives the federal government zero leverage over costs for private insurance; if anything, it incentivizes pharmaceuticals to increase private costs, since their margins for Medicare and Medicaid would be lower.

In contrast, over 80% of Germans have state-run insurance. This gives the German government a) enormous leverage and b) influence over pricing for almost all citizens.

“Within 60 days of the date of this order, the Secretary of Health and Human Services (Secretary), consistent with sections 1191 to 1198 of the Social Security Act (42 U.S.C. 1320f-1320f-7) and other applicable law, shall propose and seek comment on guidance for the Medicare Drug Price Negotiation Program for initial price applicability year 2028
