What is the worst that could happen and how many innocent people need to be killed?

I personally find that list meaningless as it is arbitrary. There are plenty of state sponsors of terrorism that should be on it, such as Pakistan, but aren't because they are American allies. It's more of a "nations the United States believes need regime change" list, and by that definition I suppose throwing Russia on the list would make sense given that Biden basically said as much. Since that is the effective meaning of the list, needless to say the nations on it don't care much for our opinion.
 
Why isn't Saudi Arabia on that list?
 
Russia should definitely be on the list, as should China.

But then again, US foreign policy and morality has always been hypocritical.

Actually the US is a terrorist state(super long list of unprovoked wars and other military criminal activities) - besides being the biggest sponsors of terrorism on the planet - nearly all islamic terrorism is sponsored by the us one way or another, then they're really big sponsors of all kinds of far-right terrorism.
