Not sure. We know that Donald likes them on the younger side, and Karoline is half the age of Melania, so given his track record it stands to reason that Trump would have moved on to someone approximating his grand-daughter.

A Tic-Toc is a ticking Tic Tac mint that explodes a burst of freshness that looks cool on TikTok videos. In Trump's mouth the freshness lasts 7 nanoseconds.
 
We know there was a camera trained on his ass the whole time just to catch 'the moment', so lets see the video the still was taken from.

Did he take it out of a pill bottle, mint container, loose in his pocket? Did Leavitt feed it to him like grapes to an emperor?

That could put this to bed, with Leavitt or not.
